Frequently asked questions about premature ejaculation

The exact cause of PE is not known. Previously it was thought to be only psychological, newer evidence has suggested a complex interaction between psychological and biological factors.

Psychological causes can include depression, early sexual experiences, guilt, etc… Biological factors may be due to abnormalities in hormone levels or neurotransmitters or inflammation/infection of the urinary tract.

The diagnosis of PE is based on history. However, if a man and his partner are not bothered by their timing of ejaculation, then no treatment is needed.

PE should be approached from a multi-modal perspective. This can include some or all of the following:

  • Psychological therapy

  • Behavioural therapy

  • Medications

Psychological therapy involves having a discussion around the situation and the potential problems that may be leading to the PE.

Psychological therapy is geared towards helping you become less nervous and working through your experience to help improve your sexual performance.

Behavioural therapy is geared towards mechanisms to help build tolerance or delay ejaculation. This can be either using the squeeze method (i.e. you or your partner squeezing the penis to prevent ejaculation) or start-stop method.

Finally, a number of medications can be used along with the aforementioned therapies. These can include pills or numbing creams or sprays. For men who may have concurrent issues with erectile dysfunction (ED), treatment involves treating the ED, as that has been shown to improve the PE.
